Elevate Health is open 9 a.m. - 5 p.m. on weekdays. Our office staff welcome you to get in touch by phone, email or secure contact form. The office hours of individual medical and wellness providers may vary.
Current patients: If you need to reach a provider after clinic hours, please call the clinic at 503-227-0350 and follow the prompts to leave a voice message for the on-call provider. They will get back to you usually within the hour. If you need emergency assistance within minutes, call 911. Do not send an online portal message for urgent issues; we do not check portal messages nights or weekends.

Elevate Health is in the southwest corner of the Pettygrove Medical Center, which comprises four buildings between NW Overton and NW Pettygrove at NW 22nd Ave. The clinic is suite 110 in building #4 (2230). Our office is easily accessible from Pettygrove, Overton and the below-structure parking lot.
If the parking garage is full, there's plenty of street parking. We'll issue you a hang tag to use during your appointment.
Driving Directions
From Northeast: Crossing the Broadway Bridge, stay to the right to take Lovejoy heading west. Turn right at NW 10th Ave, travel two blocks and turn left at NW Northrup. Head west on Northrup, and turn right on NW 22nd Ave. for one block past NW Overton.
From Southeast: Cross the Burnside Bridge and follow West Burnside to NW 21st. Turn right on 21st and go 12 blocks to NW Northrup. Turn left, and at NW 22nd turn right to find the parking entrance on your left, or metered parking on the street.
From Beaverton and other Westside locations: Take Hwy 26 east to I-405 north. At exit 2B (Everett St), exit onto NW 14th and proceed north to NW Northrup. Turn left and drive west to NW 22nd. Turn right and proceed one block to Overton. Elevate Health and the Pettygrove Medical Center will be on your left.
From Gresham and other outer Eastside locations: Take I-84 west to I-5 north, exit onto I-405 to cross the Fremont Bridge staying in the right two lanes to exit for NW Vaughn St. Now in the left turn lane, turn left onto NW 23rd Ave and proceed five blocks to turn left at NW Pettygrove. Elevate Health and the Pettygrove Medical Center will be on your right before reaching NW 22nd Ave.
Parking is free under the center, and metered street parking surrounds the vicinity.
TriMet Directions
By Streetcar: The streetcar stop most convenient to Elevate Health is located at NW 22nd on Northrup westbound. The Portland Streetcar currently runs from Legacy Good Samaritan Hospital on NW 23rd Ave to South Waterfront, via Lovejoy and Northrup, 10th and 11th Avenues, and through the Portland State University Campus.
By Bus: Line 77 stops one block from Elevate Health at NW 21st and Pettygrove both northbound and southbound. Line 15 stops at Overton on NW 23rd in both directions, just half a block from the clinic.
